Applications for residence card or registration certificate - updated forms
UK Visas and Immigration (UKVI) have updated the following application forms for EEA nationals and their family members:
- Application for a document certifying permanent residence or permanent residence card: form EEA(PR)
- Application for a registration certificate or residence card as the family member of an EEA or Swiss national: form EEA(FM)
- Application for a registration certificate or residence card for an extended family member: form EEA(EFM)
- Application for a registration certificate as an EEA national qualified person: form EEA(QP)
- Application for a derivative residence card: form DRF1
These forms are for use on or after 30 January 2015.
